I confirm that I have read and understand the information sheet for the above study. I have had sufficient time to consider the information, ask questions and have had these answered satisfactorily.
I understand that my participation is voluntary and that I am free to withdraw at any time without giving any reason, without my medical care or legal rights being affected.
I understand that I will be asked to provide a small blood sample at the start and end of the study, but can opt out any time.
I understand that relevant sections of data collected during the study may be looked at by individuals from regulatory authorities or from the NHS Trust, where it is relevant to my taking part in this research. I give permission for these individuals to have access to my records.
I agree to my General Practitioner being consulted and informed of my participation in the study.
I agree that if there are any concerns about my health during the study then my General Practitioner will be informed.
I am aware that my General Practitioner can also contact the researchers if they have any concerns about my participation.
I confirm that I am happy to provide my contact details to the course providers and researchers.
I confirm that I am happy to provide next of kin contact details in case of emergency.
I understand that if I do not fit the inclusion criteria following completion of the questionnaires, then I cannot take part in the study and the data from my questionnaires will be destroyed. However, whilst I cannot take part in the study, I can continue with the course if there are no other concerns.
